  • Definitive conclusions when retrieval is low-signal or contradictory.
  • Unverified or invented names, events, or allegations.

Used when the queried claim/name is not supported in the retrieved corpus excerpts.
INSUFFICIENT EVIDENCE
Used when retrieval quality is too weak/off-topic to answer safely.
SCOPED NAMES
Used only when multi-evidence support passes thresholds; not treated as a complete manifest.
